Faculty Reader Form

The Faculty Reader Form must be turned in along with the thesis proposal. Students should submit their completed Faculty Reader Form to Lee Price (lee@uchicago.edu).

Forms for Double Majors writing a thesis

• All double majors writing one thesis must petition to have that thesis count for both majors by submitting this form to Lee Price (lee@uchicago.edu). 
• If your thesis will be housed outside of Global Studies (i.e. you are not taking our thesis seminar), please fill out this form. 

General Petition Form

For all other petitions (including petitions to have a class count towards the major), please fill out the General Petition Form and send it to both Lee Price (lee@uchicago.edu) and Callie Maidhof (cmaidhof@uchicago.edu)
